Please fill in this form within 12 hours of your ice time. As a courtesy to your team manager, be sure it is done at least 1 hour before arriving at the arena.

Hi, my name is Dwayne Benvin and I will be the Interim Manager for the U-11 Dev (Atom Development) Team. Our Head Coach this year is Jared Kohlen and Assistant Coach Mike Rife, along with other on ice helpers.
PLAYER HEALTH SURVEY- A parent will need to complete the player health survey prior to each ice time. This can be found on the WLMHA website under the U-11 Dev tab (highlighted in RED). This needs to be completed at least 1 hour prior to the ice time. If the survey is not competed, the player will not be allowed on the ice. If you have any issues accessing the survey please let me know.
PLAYER AND PARENT CONTRACT – There will also be a player/parent contract to sign prior to going on the first ice time. This needs to be signed by any parent or family member bringing the player to the rink during tryouts and the player of course. This can be found on the main page of the website labelled Player/Parent Return to Play Contract. Please bring it with you to the rink completed. If you have someone else bring them to the rink on different days they will also need to sign it and bring it with them.
FIRST ICE TIME (Tryout) - Please arrive 30 minutes prior to the first ice time (5pm), check in at the table to collect your jersey and get changed in your designated dressing room. This is the only time you will arrive 30 minutes early, as you will then be expected to arrive no earlier than 15 minutes prior to practice. If you can’t be dressed and ready in 15 minutes, you will need to partially dress at home before you get to the rink. The dressing rooms will only be open 15 minutes prior to each ice time.
ENTRY AND EXIT TO THE RINK – Please use the far hockey entrance when arriving at the rink and obey the arrows and pathways set out by the arena staff.
DRESSING ROOMS - Each team will have 2 dressing rooms and you have been divided into those 2 dressing rooms. Please go to the same dressing room and spot each practice, as these will be the same for the first 4 practices (tryouts). Please remember to sanitize your hands immediately upon entry into the dressing room, please stay in your spots and respect the social distancing rules while at the rink. No parents shall be in the change room area. This is for players and coaches only. There will be assigned helpers to assist with skate tying or other equipment needs. Now may be a great time to start practicing these skills prior to the first ice time.
JERSEYS - Your player will be assigned a jersey for the tryout sessions. They will be responsible for their jersey during this time and it must be turned in on the last day of tryouts. Please don’t jam the jersey into the hockey bag where it can be cut, ripped or snagged on other items as we are trying to keep them in the best shape we can for games.
WATER BOTTLES – The players will NOT be able to fill their water bottles at the rink. Please ensure your player brings enough water with them for their ice time.
SHOWERS –There will be NO showers available until further notice at the rink.
DURING TRYOUTS – Parents please respect the coaches, organizers and evaluators while at the rink. Please given them the space they need to carry out their jobs. Please don’t ask them any questions about the players or how things are going. If you have any questions please direct them to me, if I don’t have an answer I will find you one.
ABSENCE – If you are not feeling well, please don’t come to practice. If you are going to miss a practice please let me know as soon as possible and the reason why.
RETURN TO PLAY DOCUMENT – Everyone has been emailed the return to play document. It can also be found on the WLMH website. Please ensure any parent or relative that are bringing players to the rink read this document. This has lots of information in it about who can come to the rink, when you can come to the rink and what to do while at the rink. Please follow the arrows and routes at the arena and respect the social distancing rules.
PARENT MEETING – There will be a parent meeting on Thursday, Sept 17 at 6pm at the rink. One parent from each family must attend.
